Overview

Although a traditional medical plan covers everyday health care expenses, if you or a family member is seriously ill or injured, you could end up with expenses and additional costs you might not expect.

You can buy extra financial protection through supplemental insurance plans, which offer a cash benefit directly to you (unless otherwise assigned) — in addition to your medical plan benefits — to help ease the financial burden. If you enroll for this coverage, you pay the full cost on a before-tax basis.

Xerox offers three different types of supplemental insurance — group accident insurance, group critical illness insurance, and group hospital indemnity insurance. You may choose to enroll in one or more than one of these plans.

Keep in mind

On their own, these plans don’t provide comprehensive medical coverage for your day-to-day health care needs. Rather, they’re intended to supplement the coverage provided by your primary medical plan.

 

Group Accident Insurance

Accident insurance through Aflac helps protect you from unexpected financial stress if you or a covered family member has an accident. If you are injured – on or off the job – this coverage pays cash benefits to help with out-of-pocket expenses. You can use this money to help pay for expenses not paid for by your medical plan (such as your deductible or coinsurance) or for anything else (such as everyday living expenses).

The amount of the benefit you receive depends on the type of injury or service you experience, including dislocations or fractures, doctor services, physical therapy, emergency room treatment, and ambulance services.

You have two options for coverage:

  • HOSPITAL CONFINEMENT (maximum of 365 days per accident, within 6 months after the accident): $150 per day
  • HOSPITAL INTENSIVE CARE (maximum of 30 days per accident, within 6 months after the accident): $150 per day
  • Up to $2,000 for dislocations and up to $3,000 for fractures
  • Many additional payments available for covered services such as emergency room or urgent care treatment, medical imaging, follow up care and physical therapy visits
  • HOSPITAL CONFINEMENT (maximum of 365 days per accident, within 6 months after the accident): $250 per day
  • HOSPITAL INTENSIVE CARE (maximum of 30 days per accident, within 6 months after the accident): $250 per day
  • Up to $3,500 for dislocations and $4,000 for fractures
  • Many additional payments available for covered services such as emergency room or urgent care treatment, medical imaging, follow up care and physical therapy visits
 

Group Critical Illness Insurance

When a serious illness strikes, critical illness insurance through Aflac can provide financial support to help you through a difficult time. If you or a covered family member experiences a covered critical illness – such as cancer, heart attack, ALS, Alzheimer’s, Parkinson’s, stroke, and more – you will be eligible for a lump-sum cash benefit, which can be used toward out-of-pocket expenses, coinsurance, or everyday living expenses such as housekeeping services, special transportation services, and child care.

Choose from two benefit coverage options:

  • $15,000 benefit amount
  • $30,000 benefit amount

The benefit is paid directly to you, unless otherwise assigned, and you can receive this benefit in addition to any other coverage, like your primary medical plan. Note that covered dependents receive 50% of the employee benefit amount.

A bonus for preventive care

To encourage you to get your preventive care, critical illness insurance also provides a $50 health screening benefit for health screening tests performed while an insured’s coverage is in force (for employee and spouse only), even if preventive care is already covered 100% by your medical plan. This benefit is not paid for dependent children.

This fixed indemnity policy may pay you a limited dollar amount if you're sick or hospitalized. You're still responsible for paying the cost of your care.

  • The payment you get isn't based on the size of your medical bill.
  • There might be a limit on how much this policy will pay each year.
  • This policy isn't a substitute for comprehensive health insurance.
  • Since this policy isn't health insurance, it doesn't have to include most Federal consumer protections that apply to health insurance.

Group Hospital Indemnity Insurance

A trip to the hospital can be stressful, and so can the bills. Even with a medical plan, you may be responsible for copays, deductibles, and other out-of-pocket costs. Offered through Aflac, hospital indemnity insurance can help offset your share of the cost by providing a cash benefit if you or a covered family member is hospitalized due to a covered sickness or for injuries received in a covered accident. Use this money to help cover hospital stays, ambulance service, surgery, and certain inpatient treatments.

You have two options for coverage:

  • HOSPITAL ADMISSION BENEFIT per confinement (once per covered sickness or accident per calendar year for each insured): $1,000
  • HOSPITAL CONFINEMENT per day for hospital ongoing care (up to 31 days): $150
  • HOSPITAL INTENSIVE CARE BENEFIT: $1,150 Day 1; $150 per day (days 2-10) for intensive care (paid in addition to daily hospital care)
  • HOSPITAL ADMISSION BENEFIT per confinement (once per covered sickness or accident per calendar year for each insured): $3,000
  • HOSPITAL CONFINEMENT per day (maximum of 31 days per confinement for each covered sickness or accident for each insured): $300
  • HOSPITAL INTENSIVE CARE BENEFIT: $3,300 Day 1; $300 per day (days 2-10 per confinement for each covered sickness or accident for each insured)
Also covers pregnancy, drug addiction, and alcoholism.

The benefit is paid directly to you, unless otherwise assigned, and you can receive this benefit in addition to any other coverage, like your primary medical plan.
